How To Sign Up:

You sign up by e-mailing coach Roark aroark1@yahoo.com

Mail in application and payment.

 

If you don’t have e-mail (?), call coach Roark at 440-610-8014


You pay by session and you own the spot in the club

(no refunds for missed times)  (you will be able to make them up if space is available)


You need to:

1   get approved by coach Roark (easy to do)

2   fill out and mail the club application and payment to coach Roark

3   fill out the medical form and bring to first session

4   fill out the waiver form and bring to first session

5   fill out the photo consent form if it’s ok to use your pictures on the web site.

6   join USATF and bring confirmation e-mail or card to first session 





The club insurance is through USATF so all vaulters and coaches must join USATF. To do so, go to https://www.usatf.org/membership/application/index.asp  (club number 18-4501)


Bottom Line:

You need to have all of your forms filled out and signed, a current USATF number (e-mail confirmation will work) and your payment at the first session.  If you don’t have these items, you get to sit and watch. No exceptions, I’m not going to risk the club if you’re not covered and I’m not a bank. Thanks for being responsible for yourself.
